Adoption Application Form

Thank you for your interest in adopting a rescue dog. This is ONLY the Mornington Peninsula Dog Rescue Incorporated adoption questionnaire, not an adoption form. The information you provide helps us to make sure the pet you adopt is the perfect fit for you and your family.

To help us make this process as fast and smooth as possible:
1. Please answer the below questions, with as much detail as possible.
2. Please upload, where indicated, pictures of your yard, fence heights, gates, and outdoor covered areas, without returned photos this does hold the process up.
3. Please upload, where indicated, pictures of existing pets, as this also helps.
4. Please upload, where indicated, ID documents

FEES
A fee will be advised and payable for any dog adopted. This covers vet work the dog will receive from Mornington Peninsula Dog Rescue Incorporated which includes:
• Sterilisation (compulsory to contract and includes puppies)
• Vaccinations - pups x 2 or 3 (depending on age) and adults x 1
• Flea and worm treatment
• Microchipping
• A thorough vet check and anything that may be needed whilst they are in care.

Please note this questionnaire may be completed by several people with a view to adopting the same pet. In this occurrence, Mornington Peninsula Dog Rescue Incorporated will review all questionnaires and choose the home best suited for the dog. Occasionally, the dog you apply for may not be the best fit, in this instance, we may recommend a dog more suited to your home and lifestyle.
