Foster Carer’s Application Form

Thank you for your interest in enquiring to join MPDRI as we endeavour to rescue dogs.

As part of our commitment to supporting our foster carers you will be required from time-to-time to participate in our in-house education program that will provide you with the understanding you need to successfully transition the dog into your home.

You will be provided with all the necessities for the dog, including a dog crate, where applicable, at no cost to yourself.

We would like you to work with your foster care co-ordinator whilst the dog is in your care.

Our foster dogs will need to be allowed access to the inside of your house during the day and they must sleep inside overnight.

The following questions are important for us to gain an understanding of what commitment you are comfortable making.

• We safely delete your information from our files once it is no longer required.
