Do you find yourself in the difficult position of no longer being able to care for your beloved pet? We understand that situations arise beyond your control and we are here to help as much as we can.
If you wish to surrender your dog to MPDRI, please fill in the Surrender form and agreement.

We Value Your Privacy:

• We take your privacy very seriously and use SSL encryption for all forms.

• We safely delete your information from our files once it is no longer required.

Surrender Fees:

• To help us provide the best care for your dog during the Surrender process, a $100 Surrender Fee is required to assist with vet checks and administration costs.
